Stem cells make 'retina in a dish'
Created by coaxing mouse embryonic stem cells into a precise three-dimensional assembly, the 'retina in a dish' is by far and away the most complex biological tissue engineered yet, scientists say.
"There's nothing like it," says Robin Ali, a human molecular geneticist at the Institute of Ophthalmology in London who was not involved in the study. "When I received the manuscript, I was stunned, I really was. I never though I'd see the day where you have recapitulation of development in a dish."
Cocktail recipe
The optic cup is brandy-snifter-shaped organ that has two distinct cell layers. The outer layer — that nearest to the brain — is made up of pigmented retinal cells that provide nutrients and support the retina. The inner layer is the retina itself, and contains several types of light-sensitive neuron, ganglion cells that conduct light information to the brain, and supporting glial cells.
To make this organ in a dish, Sasai's team grew mouse embryonic stem cells in a nutrient soup containing proteins that pushed stem cells to transform into retinal cells. The team also added a protein gel to support the cells. "It's a bandage to the tissue. Without that, cells tend to fall apart," Sasai says.
At first, the stem cells formed blobs of early retinal cells. Then, over the next week, the blobs grew and began to form a structure, seen early in eye development, called an optic vesicle. Just as it would in an embryo, the laboratory-made optic vesicle folded in on itself over the next two days to form an optic cup, with its characteristic brandy-snifter shape, double layer and the appropriate cells.
Even though the optic cups look and develop like the real thing, "there may be differences between the synthetic retina and what happens normally," Ali says.
The eyes have it
Synthetic human retinas could provide a source of cells to treat conditions such as retinitis pigmentosa, in which the retina's light-sensing cells atrophy, eventually leading to blindness. In 2006, Ali's team found that retinal cells from newborn mice work when transplanted into older mice5. Synthetic retinas, he says, "provide a much more attractive, more practical source of cells".
David Gamm, a stem-cell biologist at the University of Wisconsin, Madison, says that transplanting entire layers of eye tissue, rather than individual retinal cells, could help people with widespread retinal damage. But, he adds, diseases such as late-stage glaucoma, in which the wiring between the retina and brain is damaged, will be much tougher to fix.
When and whether such therapies will make it to patients is impossible to predict. However, in the nearer term, synthetic retinas will be useful for unpicking the molecular defects behind eye diseases, and finding treatments for them, Sasai says. Retinas created from reprogrammed stem cells from patients with eye diseases could, for instance, be used to screen drugs or test gene therapies, Ali says.
Robert Lanza, chief scientific officer of the biotechnology company Advanced Cell Technology, based in Santa Monica, California, says the paper has implications far beyond treating and modelling eye diseases. The research shows that embryonic stem cells, given the right physical and chemical surroundings, can spontaneously transform into intricate tissues. "Stem cells are smart," Lanza says. "This is just the tip of the iceberg. Hopefully it's the beginning of an important new phase of stem-cell research."

Ανέπτυξαν βολβούς ματιού από βλαστοκύτταρα
Μία ομάδα Ιαπώνων επιστημόνων τοποθέτησε βλαστοκύτταρα από έμβρυα ποντικιών σε ένα δοχείο με χημικά, και δημιούργησαν βολβούς ματιών με αμφιβληστροειδή χιτώνα σε πρώιμο στάδιο.
Το επιστημονικό περιοδικό Nature, που δημοσίευσε την εν λόγω έρευνα, χαρακτήρισε τις κενές, σε σχήμα βολβού, δομές ως «εξαιρετικές».
Ο ερευνητής Δρ. Yoshiki Sasai δήλωσε πως τα αποτελέσματα δίνουν σημαντικές ελπίδες για τη θεραπεία διαταραχών που σχετίζονται με την όραση, όπως της μελαγχρωστικής αμφιβληστροειδοπάθειας

Study Of Umbilical Cord Blood-Derived Stem Cells For Lupus Therapy
The beneficial results were reported in a study by Taiwanese researchers published in the current issue of Cell Transplantation (20:2), freely available on-line here.
According to corresponding author Dr. Oscar K. Lee of the National Yang-Ming University School of Medicine, MSCs have been shown to possess immune-modulatory capabilities and can alleviate immune responses by inhibiting inflammation as well as the function of mature and immature immune system T cells. Seeking to explore the therapeutic effects of uMSCs in treating LN, their study transplanted umbilical cord blood-derived stem cells into mice modeled with systemic immune diseases closely resembling SLE in humans.
"We found that uMSC transplantation markedly delayed the deterioration of renal function, reduced certain antibody levels, alleviated changes in renal pathology and the development of proteinuria - the presence of excess protein serum in the urine and a sign of renal damage," said Dr. Lee.
The positive difference in survival rate for mice treated at two months of age compared with mice treated at six months of age, led the researchers to conclude that early uMSC transplantation may be most efficacious. The researchers also deduced that their findings favored the use of allogenic (other-donated) rather than autologous (self-donated) MSCs for SLE treatment, which would make sense with an autoimmune disorder.
"The therapeutic effects demonstrated in this pre-clinical study support further exploration of the possibility of using uMSCs from mismatched donors in LN treatment," concluded Dr. Lee.
"The ability of uMSCs to reduce inflammation means that they are likely to be of use in the treatment of autoimmune disorders and this study supports that reasoning and, in this case, also advocates the use of non-self cells," said Dr. David Eve, associate editor of Cell Transplantation and an instructor at the University of South Florida Center of Excellence for Aging and Brain Repair.

Testing power of cord blood
Researchers are seeking ethics approval for a trial at Melbourne's Monash Medical Centre that would offer the treatment to 20 children to determine whether it can cure or relieve the symptoms of cerebral palsy.
Professor Joanne Kurtzberg, of North Carolina's Duke University, has reinfused the cord blood of about 250 children with cerebral palsy over the past five years - but has published no scientific data on its effectiveness. While Professor Kurtzberg has established that the treatment is safe and feasible, Australian researchers want to assess the impact on the brains of children with cerebral palsy.
Since having his cord blood reinfused four months ago, the Archers say movement on Zac's right side is improving and he is now having about 20 partial (or focal) seizures a day, compared with 100 previously.
The blood that remains in a baby's umbilical cord and placenta after birth is rich in stem cells, which are able to develop into other cells in the body. Southern Health's director of obstetrics, Professor Euan Wallace, said animal studies suggested that the stem cells in cord blood could repair or assist in the repair of brain injuries.
One theory is that stem cells in cord blood go to the damaged area and recruit cells within the brain to repair it, but the exact mechanism is unknown.
''If it doesn't work, then Australian families shouldn't be spending $40,000 going to the US. If it does work, then we need to know about it because, for the first time, we would have a treatment that actually reverses cerebral palsy. That would be astonishing,'' Professor Wallace said.
He said all the children in the trial - expected to start this year - would be offered the treatment. ''It's a slightly unusual trial design - all of the children will be offered the blood and a full neurological assessment, and then randomised to have the cord blood either straight away or in three months before being reassessed,'' Professor Wallace said.
''That way you are not withholding treatment from any child, but you have a small core of children where you can see whether the cells are working or not.''
Cord blood is used to treat blood disorders including leukaemia in donor recipients - and a public bank in Australia collects it for that purpose. Cord blood also shows promise in the emerging field of regenerative medicine to repair damaged or diseased tissues.
Children in the trial may have been able to reclaim their cord blood from the public bank, or a private bank that retains it for their exclusive use.
The medical director of private cord blood bank Cell Care, Associate Professor Mark Kirkland, said the trial would be one of the first to assess the efficacy of cord blood in human disease.
''For years researchers have been writing in the scientific literature about the therapeutic potential of cord blood … and people have started collecting it on that basis,'' he said.
''This is the first of what I think will be a large number of trials looking at applying in the real world what people have been observing in the laboratory for many years, which is that cord blood is a unique source of cells for a whole range of therapies.''
Cord blood stem cells have also been proposed as a treatment for conditions including spinal cord injury, diabetes, liver and heart disease.

Βλαστοκύτταρα και αντιγήρανση
Τα τελευταία χρόνια η χρήση των βλαστοκυττάρων αντικατέστησε με επιτυχία τους ινοβλάστες και έτσι τα βλαστοκύτταρα εμφυτευόμενα στο πρόσωπο έχουν την ικανότητα της παραγωγής των στοιχείων του δέρματος, η απώλεια των οποίων οδηγεί στη γήρανση. Σήμερα η λήψη βλαστοκυττάρων για την αποκατάσταση της φθοράς του δέρματος γίνεται από τον λιπώδη ιστό. Ο λιπώδης ιστός αποτελεί την πλουσιότερη πηγή λήψης βλαστοκυττάρων μεσεγχυματικού τύπου, τα οποία παράγουν τις σημαντικότερες πρωτεΐνες του δέρματος. Η λήψη των βλαστοκυττάρων από τον λιπώδη ιστό γίνεται με λιποαναρρόφηση και η χρήση τους μπορεί να γίνει άμεσα ή να κρυοσυντηρηθούν και να χρησιμοποιηθούν στο μέλλον. Η χορήγησή τους γίνεται τοπικά στις ρυτίδες και για άμεσο αποτέλεσμα χορηγούνται συγχρόνως με μια ποσότητα λίπους ανάλογα με το βάθος της ρυτίδας που θέλουμε να γεμίσουμε. Ετσι το λίπος δίνει ένα άμεσο αποτέλεσμα γεμίζοντας τη ρυτίδα και τα βλαστοκύτταρα αρχίζουν την παραγωγή των φυσιολογικών συστατικών του δέρματος, τα οποία δίνουν ένα μόνιμο αποτέλεσμα. Το λίπος μετά από ορισμένο χρονικό διάστημα απορροφάται κι έτσι παραμένει η λειτουργική δραστηριότητα των βλαστοκυττάρων.
Τα μεσεγχυματικά κύτταρα σχηματίζουν το στηρικτικό σύστημα του σώματός μας, αλλά και το σκελετό όλων των οργάνων επάνω στον οποίο στηρίζεται το λειτουργικό μέρος τους. Τα οστά, οι μύες, οι χόνδροι, τα αγγεία και οι τένοντες δημιουργούνται από τα μεσεγχυματικά κύτταρα. Με την πάροδο της ηλικίας η ανανέωση αυτών των οργάνων επιβραδύνεται, με αποτέλεσμα την έκπτωση της λειτουργίας τους. Χαρακτηριστικό παράδειγμα η εμφάνιση της οστεοπόρωσης, λόγω αδυναμίας των οστεοκυττάρων να παραγάγουν το σημαντικότερο συστατικό του σκελετού που είναι το κολλαγόνο. Ως αποτέλεσμα, τα ανόργανα άλατα που δίνουν τη σκληρότητα στα οστά μας δεν μπορούν να βρουν το φυσικό τους υπόστρωμα για να εναποτεθούν και γίνονται εύθραυστα.
Η χορήγηση μεσεγχυματικού τύπου βλαστοκυττάρων οδηγεί στον εμπλουτισμό των οργάνων με νεαρά κύτταρα, τα οποία έχουν πλήρη λειτουργική δραστηριότητα, προκαλώντας έτσι μια γενική ανανέωση των ιστών του σώματός μας.
Ολοι οι ιστοί διαθέτουν βλαστοκύτταρα στο υπόστρωμά τους, το απόθεμά τους όμως εξαντλείται με την πάροδο της ηλικίας. Στον ενήλικα τα βλαστοκύτταρα παραμένουν αποθηκευμένα σε συγκεκριμένες θέσεις, εν υπνώσει, και απαιτούνται ειδικές μέθοδοι συλλογής και διαχωρισμού. Ο λιπώδης ιστός διακρίνεται για τον μεγάλο αριθμό των βλαστοκυττάρων που περιέχει. Σε συνδυασμό και με την εύκολη λήψη τους αποτελεί σήμερα την πολυτιμότερη πηγή λήψης βλαστοκυττάρων. Πέρα όμως από την ανανέωση των οργάνων, τα βλαστοκύτταρα επιδρούν στο ανοσοποιητικό μας σύστημα, ασκώντας ρυθμιστική δράση και προστατεύοντάς το από υπερβολικές αντιδράσεις, οι οποίες οδηγούν στην εμφάνιση των αυτοάνοσων παθήσεων. Οι κακώσεις του σώματος, είτε είναι εμφανείς, εξωτερικές είτε εσωτερικές, προκαλούν μεγάλες καταστροφές στα όργανα. Η χορήγηση μεσεγχυματικών κυττάρων οδηγεί σε ταχύτερη αποκατάσταση της λειτουργίας τους. Η χρήση τους ως μεταφορέων φαρμάκων και η κατευθυνόμενη χορήγησή τους σε όργανα αποτελεί μια νέα αποτελεσματική μέθοδο θεραπείας.

Recovering from cancer thanks to cells from her umbilical cord
Because of the stem cells transplant from her own umbilical cord, she has experienced appreciable improvements that make her parents feel more hopeful about the recovery of their daughter.
Although the little girl was born healthy on February 9, 2007, her parents decided to freeze her umbilical cord to a private entity to preserve this way her stem cells.This decision was providential, because at 22 months old Alba suffered from a tumor and even if she had surgery ,her prognosis were not very good.
Luis Madero, head of the hospital Oncohematology Service, told El Pais that the illness of the little girl had a peculiarity. Being a solid tumor, was likely to be treated with stem cells transplant. If it was a leukemia,a cancer more common in children, this treatment option would have been more problematic, since this type of cancer usually has a genetic origin, so the tumor would already have been in the umbilical cord blood. Teresa Molina, the mother of the child, argues that from the moment they decided to freeze the umbilical cord of Alba, unknowingly they signed a “life insurance” for her.
